Once every year, you will meet with your Doctor to have a conversation about your health objectives and goals. 

During the rest of the year, you will discuss how the plan of action is going and what adjustments may be needed.

In between those encounters there might be times where a conversation may be needed for various reasons, your Doctor will be there to assist you.
